How to register your pregnancy with a midwife has changed within Leicester, Leicestershire and Rutland (LLR)
To self-refer please visit Health for Under 5's and choose LLR as the area (even if you wish to give birth in a Nottingham or Derby Hospital, the midwives will sort this out).

- You will be given a booking appointment for when you are 8 – 10 weeks pregnant, the midwife will inform you of where this will take place.
- If you are more than 12 weeks pregnant, or are unable to complete this form, please call the Community Midwifery Office on 0116 258 6111 (press option 5).
