What to do when the Surgery is closed.
GP Appointment Requests can be submitted Monday to Friday 8.00am to 6.30pm
Receptionists from the 22nd September 2025 will no longer book GP appointments, all requests must go through the medical triage form (https://florey.accurx.com/p/C82064).
For any GP appointments requests ranging from acute to chronic problems requiring assessment please submit your request via our online triage form (https://florey.accurx.com/p/C82064).
We will review your medical request by the end of the next business day.
If you require assistance completing the triage form, please call reception who will help to accommodate your needs.
Nurse and Wider Health Team Appointments
Please call reception to book appointments for these members of the team between 9.30 and 5.30 Monday to Friday.
Home Visits
Home visits can be arranged for housebound patients when for medical reasons they are unable to attend the surgery. Patients are requested to telephone before 11.00am if a home visit is required.
Please give the receptionist as much information as possible to enable the doctor to triage the urgency of the visit. Visits are carried out by GP's and our designated home visiting service clinicians.

Please be aware there is a form to fill in and we will need identification to register you for example- Passport, Picture Driving License, Utility Bill/Bank Statement dated within 3 months (not mobile phone).

Chaperones
If you require a chaperone for any appointment, please make this clear to the reception staff at the time of booking your appointment. Arrangements can be made to ensure that a chaperone is present during the consultation. Our chaperones are fully trained.
